But recovery depends on several factors — cancer location, baseline erectile function, age, vascular health, time, and structured rehabilitation. The starting point matters. What happens next matters just as much.

Why erectile dysfunction happens after robotic prostatectomy

Even with a robotic-assisted approach and meticulous technique, those nerves can be stretched, bruised, or temporarily disrupted during surgery.

In practice, this means a structured plan rather than a single medication — and it should begin as soon as healing allows, typically within the first four to six weeks after surgery.

Progress tends to come in stages rather than all at once:

Return of penile sensation — often one of the earliest signs of nerve recovery
Partial or morning erections — an encouraging early indicator that nerve function is returning
Erections with tablets or a vacuum device — usable function, even if not yet spontaneous
Firmer erections with combination treatment — improvement continues as recovery progresses
Gradual improvement in confidence and spontaneity — often the final milestone, and may take the longest
